Use the Excel game label template with Avery 8163 labels. Enter the information only in the first block of cells and let the spreadsheet do the rest. These labels work best when printed in color.

All managers are required to register with USA Hockey, complete Safe Sport training, and submit to a background check. These are required biannually.
USA Hockey Registration - Register as an Ice Manager/Volunteer. There is no cost for this registration. Take note of your confirmation number, you will need it for the next two steps.
SafeSport Training - You must first be a member of USA Hockey. Your membership covers the cost of the training.
Background Screening - You must first register with USA Hockey. There is a $30 fee for the background screening. You can request reimbursement from OYHA by emailing your receipt to treasurer@ozaukeehockey.com.
CERTIFICATIONS - SafeSport, Background Checks, and Coaching Certifications, as well as questions related to these items should be sent to registrar@ozaukeehockey.com.
SCHEDULE CHANGES - All schedule changes for home games must be sent to Chris Donovan at ozicecenter@gmail.com.
REFEREE REQUESTS - All questions, requests, or changes that affect referee scheduling must be sent to Mike Hester at refcoordinator@ozaukeehockey.com.
WEBSITE - All website related questions can be sent to OYHA webmaster, Steve Callen, at webmaster@ozaukeehockey.com.
